Everything in the online store is in stock and ready to ship.
All orders must be placed through our secure online ordering system.
No orders will be accepted by phone, fax or mail.
No International orders.

There is a 12 hour window to make changes to your order before it is placed in our packing rotation.

We accept all major credit cards and PayPal. Your credit card purchase will reflect THE MODERN FORM as the payee for your The Modern Form purchase.
Shipping Information
We ship all orders Monday through Friday. Orders are processed on the following business day.
Please make sure your shipping address is complete. A wrong address can cause a shipping delivery delay or an undeliverable package.
Shipping Methods:
We ship to anywhere in the Continental United States via USPS Priority Mail with signature confirmation delivery or Fed-X depending on the destination.
Warranty
There is a money-back guarantee for 30 days from original purchase date against defects in materials and craftsmanship.
Returns Policy
- The Modern Form does not accept returns on used items. If an item is returned in used condition it will be refused and no credit will be issued.
- Return authorization is required. Simply contact us through our contact form for return authorization. Please include your invoice number with your request and the reason you would like to return it.
- Returns will be accepted for 15 days from the day the item is received by the customer.
- Returned items must be sent prepaid with an insured signature delivery confirmation by a traceable carrier such as USPS, UPS or Federal Express.
- The Modern Form cannot be responsible for any returned packages lost by the carrier. If you add a signature delivery confirmation to your return, your package will be signed for,trackable, and will serve as evidence of shipping.
- Returned items must be returned in the same packaging used to ship the item to the customer. The Modern Form is not responsible for products that are damaged during shipping back to us if other packaging is used. If a return is received damaged then a full credit can not be issued.
- The Modern Form will not accept any COD packages.
- Once we receive the returned item, credit will be issued to the credit card or PayPal account originally used to place the order.
- Returns for credit must include the original invoice number that was provided at time of purchase and is also found on the printed invoice.
- All returns are subject to a 30% restocking fee.
Exchange Policy
- The Modern Form does NOT accept exchanges for any merchandise.

All of our customer information is kept private and NEVER shared with outside companies or 3rd parties. We use SSL encryption when processing contact information. You will know that SSL encryption is being used when the "padlock" icon in the corner of your browser is "closed" and the URL begins with "https". No credit card information is ever stored in our database since all of the payment transactions are handled by VeriSign or PayPal. Both are trusted and secure ecommerce payment systems.
